Annual Preventive Plan Costs

· Adult total annual cost: $320

(saves $100 off our normal fees)

· Child (under 14) total annual cost: $280

(saves $200 off our normal fees)

Our plan includes:

· 2 preventive exams

· 2 cleanings (prophylaxis or periodontal maintenance)

· 2 oral cancer screenings

· All routine diagnostic x-rays as needed

· 15% off additional cleanings, dental sealants, fillings, core buildups, oral surgery, emergency exams, and x-rays

· 15% off crowns, veneers, periodontics, dentures, partials, implants, and root canals

· $500 off full treatment invisible braces

· $250 off express invisible braces

Patients agree that Comprehensive Dentistry’s fees stated must be paid at the time services are rendered. Any service not paid for at the time of service will be billed at usual and customary fees. Plan fees are valid only when paid at the time of enrollment. All family members must reside in the same household. This is not an insurance product.

Program Guidelines:

Plan is for patients who do not have an employer-sponsored dental plan.

Patient’s portion of bill is due the day of service.

The plan is non-refundable

CareCredit can not be used

Patient must have comprehensive exam and full mouth series (FMX) of x-rays to be eligible to join

I really cant say enough about this group! They are out of my insurance network but time and time again they prove its so worth it. They value my time, educate me and are very professional. I have been a patient of Dr. Baronas for many years, and have worn a partial plate for most of that time. About four years ago, after a thorough discussion with him, I decided to have an implant placed. Just recently, I had a second one. In both cases, the two-part procedure was not unpleasant as he uses the most up-to-date equipment and techniques. It has been well worth the time and expense to eliminate the problems of a partial plate.
